What to Expect During Your First Dentist Appointment

As a new patient at Fairview Dentistry in St. Thomas, your initial dentist appointment will include a consultation with one of our dentists.

During your first dentist appointment with us, our team will take digital x-rays and photographs for our records, and will review your past dental and medical history. This will help us get a complete picture of your oral health, which in turn will inform your treatment process.

Your dentist will also perform a quick oral examination to present a diagnosis of any dental problems you are experiencing. Based on this examination, the dentist will recommend the appropriate treatment options for your dental care.

At your first appointment, please provide:

  • Your Health Card Number
  • A list of medications you are presently taking
  • If you have medical or dental insurance, please bring your insurance card or number. This will save time and allow us to help you process any claims or pre-determinations.

*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.

We want to provide you with the dental services that you need and while that means a bit of paperwork (although we do use a wipe-able form and scan the information to save trees) to help us care for your dental health.

Sedation Appointments

Please provide the following information at the time of your consultation:

  • Your referral slip, if applicable
  • A list of medications you are presently taking
  • If you have medical or dental insurance, bring the necessary completed forms.
  • Please let us know if you have a medical condition that may be of concern prior to surgery (e.g., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.).
  • Also, be sure to advise us if you are on any medication (e.g., heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for pre-med.)
